Training Mission Number Forms
ICS-201.b- used any time a new training or event is planned, that has never been done before (201's from previous years can be re-used if the content of the event is unchanged, but inform Volunteer Coordinator of updated time/date/location.)
ICS-214- records details of notable activities on any mission response or training exercise. These logs provide basic incident activity documentation, and a reference for any after-action reporting.
Activity Report (Sign-In Sheet)- used and submitted to Volunteer Coordinator after any volunteer activity
Training Mission Number Request- only submitted once annually. This version of EMD-079 will be phased out after 2024 and replaced with a simplified form provided by WA EMD.
Training Mission Number Schedule (Spreadsheet)- only submitted once annually
Reimbursement forms that may be utilized by registered Emergency Worker Volunteers after a qualifying expense has been made on a mission or during approved training mission number activities:

Property Loss or Damage Reimbursement Form
Medical Expense Claim Form This form is required to reimburse volunteers for any medical expenses incurred after an injury while serving as a volunteer. This form must be submitted along with an activity report (sign-in sheet) signed by the unit lead, verifying the date, time and location of the incident. Additionally, please provide proof of payment for any medical expenses you intend to seek reimbursement for.
Reimbursement funds are provided by the State of Washington, not Skagit County. In order to file a claim, claimants must have a Statewide Vendor Number (SVN), which is obtained via the Washington State Department of Enterprise Services.

SCSO SAR After Action Report (AAR) Form. If you'd like to print and hand-deliver this form, to ensure anonymity, you may print this fillable-PDF version. Currently, this form is designed for SCSO SAR units, however a general Emergency Worker AAR form will be uploaded in the near future.
